Life-Stage Appropriate Care for Young Adults with Type 1 Diabetes (#7)

Young adults with type 1 diabetes mellitus are a high risk group with sub-optimal glycaemic outcomes when compared to older adults with type 1 diabetes. Management is challenging due to a number of factors including psychosocial issues, an increased incidence of psychiatric illness and impaired quality of life. One of the major challenges facing practitioners is maintaining the engagement of young adults with diabetes services given the high rate of loss to follow-up following transition from paediatric care. This session focuses on diabetes service delivery to this vulnerable group, with attention to addressing barriers and enablers to provide life-stage appropriate care.
